1. SCOPE, AND HOW THIS POLICY RELATES TO HIPAA

This Privacy Policy explains how Skincare2Me, LLC d/b/a NuVitality4You (“NuVitality4You,” “we,” “us,” or “our”) collects, uses, and shares personal information through www.nuvitality4you.com, our online forms and booking tools, and our email, text, and social media communications (together, the “Site”).

When you become a patient, the health information we create or receive in order to provide you care is “protected health information” governed by the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. That information is handled under our HIPAA Notice of Privacy Practices, which we give to patients separately and which controls if it ever conflicts with this policy. This Privacy Policy covers information that is not protected health information, such as information from Site visitors and prospective clients.

2. INFORMATION WE COLLECT

  • Information you give us: your name, email address, phone number, mailing address, state of residence, appointment details, the services you are interested in, your wellness goals, and anything else you choose to share in a form, email, text, call, or social media message. Some of this may relate to your health.

  • Information collected automatically: IP address, general location (city/state), browser and device type, pages viewed, links clicked, the date and time of your visit, and the website that referred you. We and our service providers collect this using cookies and similar technologies.

  • Payment information: payments are processed by third-party payment processors. We do not store your full card number on our systems.

  • Information from others: for example, a referral from someone you know, or information from scheduling, advertising, and social media platforms about how you found us.

3. HOW WE USE INFORMATION

  • To respond to your questions, schedule and manage appointments, and send you the forms needed before your visit.

  • To provide, coordinate, and improve our services, and to determine whether services are available in your state.

  • To send service-related emails and texts, and — where you have opted in — educational and promotional emails and texts.

  • To understand how the Site is used, measure our marketing, and improve the Site.

  • To process payments, prevent fraud, protect the security of the Site, comply with law, and enforce our Terms.

4. HOW WE SHARE INFORMATION

We do not sell your personal information, and we do not share it with third parties for their own marketing. We share personal information only as follows:

  • Service providers who work on our behalf under confidentiality obligations — for example, website hosting, scheduling and video conferencing, customer relationship management and email/text messaging, electronic signature, patient portal and health record software, payment processing, and analytics.

  • Licensed healthcare providers, laboratories, and pharmacies involved in evaluating or delivering services you request.

  • Legal and safety reasons — when required by law, subpoena, or court order, or when needed to protect the rights, property, or safety of our patients, the public, or the Company.

  • Business transfers — if the Company is involved in a merger, sale, or transfer of assets, subject to applicable law.

  • With your direction or consent.

Text messaging: mobile phone numbers, text messaging opt-in data, and consent are not shared with or sold to any third parties or affiliates for marketing or promotional purposes. They are shared only with the service providers that help us deliver our text messages.

5. COOKIES, ANALYTICS, AND ADVERTISING

We use cookies and similar technologies to operate the Site, remember your preferences, and understand how visitors use the Site. We use Google Analytics to collect usage information; you can learn how Google uses this data and opt out at https://tools.google.com/dlpage/gaoptout/. You can set your browser to refuse cookies, though parts of the Site may not work as intended. The Site does not currently respond to browser “Do Not Track” signals.

6. YOUR CHOICES

  • Marketing emails: click “unsubscribe” in any marketing email.

  • Text messages: reply STOP to any text to opt out; reply HELP for help. Message and data rates may apply; message frequency varies.

  • Access, correction, and deletion: you may ask us to access, correct, or delete the personal information we hold about you by emailing info@nuvitality4you.com. We will respond as required by applicable law. We may need to verify your identity, and we may need to keep certain information — for example, medical records we are legally required to retain. Patients’ rights regarding their health records are described in our HIPAA Notice of Privacy Practices.

  • State privacy rights: depending on where you live, you may have additional rights under your state’s privacy laws, including the right to appeal a decision we make about your request. We will not discriminate against you for exercising your rights.

8. HOW WE PROTECT AND RETAIN INFORMATION

We use administrative, technical, and physical safeguards designed to protect personal information, and we use secure, access-controlled systems for health information. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Please do not send detailed health information by standard email or text message; use our secure patient portal instead.

We keep personal information for as long as needed for the purposes described in this policy, and as required by law. Medical records are kept for the period required by applicable state and federal law.

9. CHILDREN

The Site and our services are intended for adults and are not directed to anyone under 21. We do not knowingly collect personal information from children. If you believe a child has provided us with personal information, please contact us and we will delete it.

10. THIRD-PARTY SITES

The Site may link to websites and tools operated by others. This Privacy Policy does not apply to them, and we encourage you to read their privacy policies.

11. CHANGES TO THIS POLICY

We may update this Privacy Policy from time to time. We will post the updated policy on the Site and revise the effective date above. Material changes will be highlighted on the Site or communicated to you directly where appropriate.
