1. INTRODUCTION AND ACCEPTANCE
Skincare2Me, LLC d/b/a NuVitality4You (“NuVitality4You,” “Company,” “we,” “us,” or “our”) operates the website located at www.nuvitality4you.com and its related pages, forms, and booking tools (the “Site”). These Terms of Use (“Terms”) govern your access to and use of the Site. By accessing or using the Site, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Site.
“You” and “your” refer to any individual or entity that accesses or uses the Site. Additional terms may apply to specific services, programs, or purchases; where those terms conflict with these Terms, the more specific terms control for that service.
2. ELIGIBILITY
The Site is intended for adults. You must be at least 21 years old to use the Site, submit a form, or book an appointment. Clinical services are offered only to adults who meet our eligibility criteria and only in states where our affiliated providers are licensed to practice.
3. NOT MEDICAL ADVICE; NO PROVIDER-PATIENT RELATIONSHIP; EMERGENCIES
The content on the Site — including articles, guides, blog posts, videos, social media content, and descriptions of peptides, hormones, diagnostic testing, and other therapies — is provided for general educational and informational purposes only. It is not medical advice, diagnosis, or treatment, and it is not a substitute for an evaluation by a licensed healthcare professional who knows your medical history.
Using the Site, submitting a form, booking or attending a complimentary Wellness Assessment, or communicating with us by email, phone, or text does not by itself create a provider-patient relationship. A provider-patient relationship is formed only after you complete our intake and consent documents and a licensed provider accepts you as a patient.
Never disregard professional medical advice or delay seeking it because of something you read on the Site. Do not use the Site, email, or text messages for urgent medical needs. If you think you may be having a medical emergency, call 911 or go to the nearest emergency room immediately.
4. OUR SERVICES, TELEHEALTH, AND REGULATORY STATEMENTS

Provider evaluation required. Any therapy, prescription, or clinical recommendation is provided only after review by a licensed healthcare provider, who decides in his or her independent professional judgment whether a therapy is appropriate for you. Submitting a request or payment does not guarantee that a therapy will be prescribed.

Telehealth. Some services are delivered by telemedicine. Telehealth services are available only in states where our affiliated providers are licensed, and availability may change.

FDA statements. Statements on the Site have not been evaluated by the U.S. Food and Drug Administration. Some therapies discussed on the Site, including certain peptides and compounded medications, are not FDA-approved, or are prescribed for uses other than those approved by the FDA. Compounded medications are not reviewed by the FDA for safety, effectiveness, or quality. Information on the Site is not intended to diagnose, treat, cure, or prevent any disease.

Results vary. Individual results differ and are not guaranteed. Any testimonials, stories, or examples reflect individual experiences and are not a promise of what you will experience. Individuals who share content about us in exchange for discounts, products, or commissions are required to disclose that relationship.

Service availability. Some services described on the Site may not yet be available or may be offered only in certain locations. Descriptions of services are not an offer to provide them in every location.
5. APPOINTMENTS, PURCHASES, AND PAYMENTS
Appointments are booked through third-party scheduling tools linked from the Site. Prices, fees, and any applicable payment-method fees are disclosed to you before you pay. We do not bill insurance unless we tell you otherwise in writing.
6. EMAIL AND TEXT MESSAGE COMMUNICATIONS
By providing your email address, you agree that we may send you messages about your inquiries, appointments, and account. If you opt in to marketing emails, you can unsubscribe at any time using the link in any marketing email.
Text messaging (SMS) terms. If you give us your mobile number and opt in, you agree to receive text messages from NuVitality4You at that number, which may include appointment reminders, follow-ups about your inquiry, educational content, and promotional messages. Consent to receive marketing text messages is not a condition of purchasing any goods or services. Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out; reply HELP for help, or contact us at info@nuvitality4you.com or (813) 731-1863. Carriers are not liable for delayed or undelivered messages. Text messages and standard email are not secure methods for sharing detailed health information; please use our secure patient portal for that purpose.
7. PRIVACY
Personal information collected through the Site is handled as described in our Privacy Policy below. Health information we receive in the course of providing clinical services is handled as described in our HIPAA Notice of Privacy Practices, which is provided to patients separately.
8. COPYRIGHT AND TRADEMARKS
All content on the Site — including its design, text, graphics, images, logos, guides, videos, page headers, and software, and the selection and arrangement of those materials (the “Content”) — is owned by or licensed to the Company and is protected by United States and international copyright, trademark, and other intellectual property laws. NuVitality4You and its logos are trademarks of the Company.
You may view, download, and print Content for your own personal, non-commercial use, provided that you keep all copyright and proprietary notices intact and do not modify the Content. Any other use — including copying, republishing, distributing, selling, scraping, or creating derivative works — requires our prior written permission. To request permission, email info@nuvitality4you.com. You may link to the Site without permission, provided the link does not suggest that we endorse you or your products. All rights not expressly granted are reserved.
9. CONTENT YOU SUBMIT
If you submit reviews, comments, photographs, or other materials to us through the Site, email, or social media (“Submissions”), you represent that you are at least 21, that you own or have the right to share the Submission, and that it is truthful. You grant the Company a non-exclusive, royalty-free, worldwide license to use, reproduce, edit, and publish the Submission in connection with our business, including on the Site and our social media pages. You may ask us at any time to stop future use of your Submission by emailing info@nuvitality4you.com.
We will not publish a patient’s photograph, testimonial, or any information that identifies someone as our patient without that patient’s separate signed written authorization. Please do not post your own health information in public comments or reviews.
10. PROHIBITED ACTIVITY
You agree not to: use the Site for any unlawful purpose; submit false information or impersonate another person; post or transmit material that is unlawful, infringing, threatening, harassing, defamatory, obscene, or otherwise objectionable; upload viruses or other harmful code; attempt to gain unauthorized access to the Site, its systems, or other users’ information; scrape, harvest, or collect data from the Site by automated means; interfere with the normal operation of the Site; or use the Site to obtain prescription products by misrepresenting your identity, health history, or location.
11. EXTERNAL LINKS AND THIRD-PARTY TOOLS
The Site may link to or embed third-party websites and tools, such as scheduling, payment, patient portal, laboratory, and social media services. We provide these for convenience and do not control, endorse, or guarantee them. Your use of a third-party site or tool is governed by that third party’s own terms and privacy policy, which we encourage you to review.
12. DISCLAIMER OF WARRANTIES
The Site and all Content are provided on an “as is” and “as available” basis. We work to keep information accurate and current, but to the fullest extent permitted by law, the Company disclaims all warranties, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, secure, or error-free.
13. LIMITATION OF LIABILITY
To the fullest extent permitted by law, the Company and its owners, officers, employees, contractors, and affiliated providers will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of data, business, or profits, arising out of or relating to your use of, or inability to use, the Site, its Content, or any third-party site linked from it, whether based on contract, tort (including negligence), or any other legal theory, even if we have been advised of the possibility of such damages. Our total liability for any claim relating to the Site will not exceed one hundred dollars ($100) or the amount you paid us through the Site in the 12 months before the claim, if greater.
This section applies to your use of the Site. It does not limit any liability that cannot be limited under applicable law, and it does not govern clinical services, which are covered by the consent and treatment documents you sign as a patient.
14. INDEMNIFICATION
You agree to defend, indemnify, and hold harmless the Company and its owners, officers, employees, and contractors from any claims, damages, liabilities, and expenses (including reasonable attorneys’ fees) arising out of your violation of these Terms, your misuse of the Site, or your Submissions.
15. SUSPENSION AND TERMINATION
We may suspend or terminate your access to the Site, in whole or in part, at any time and without notice if you violate these Terms or if your use is harmful to us or to another user. Sections that by their nature should survive termination will survive.
16. GOVERNING LAW AND VENUE
These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-law rules. Any dispute arising out of or relating to these Terms or the Site will be brought exclusively in the state or federal courts located in Hillsborough County, Florida, and you consent to the jurisdiction of those courts.
17. CHANGES TO THESE TERMS
We may update these Terms from time to time. When we do, we will post the revised Terms on the Site and update the effective date above. Your continued use of the Site after the revised Terms are posted means you accept them.
18. GENERAL
If any provision of these Terms is found unenforceable, the remaining provisions will remain in effect. Our failure to enforce a provision is not a waiver of it. These Terms, together with the Privacy Policy and any additional terms that apply to a specific service, are the entire agreement between you and us regarding the Site.
